Integrated Assessment (MH/SA)

Service Description

This assessment is typically completed over the course of 1-2 (60 minute) sessions. Information obtained during the assessment process includes: Mental health symptoms you are experiencing, psychiatric, medical, and substance use history for yourself (and your family), social, employment, and educational background, as well as legal status. This is an opportunity for me to learn about you as a whole person. This is also a time for us to discuss what the best approach is moving forward. We will discuss treatment goals and objectives, and develop a treatment plan together.
